The registration form is available as an Acrobat PDF file here. You will need Acrobat reader to view and print it. Please remember to send registration forms to the address on the form and not to AFLC headquarters. Registration forms should be in the mail no later than June 2 to ensure that they will arrive on time.

You will need to print page 2 of the registration form if you are planning to bring your children and send them to Vacation Bible School or use the host-provided childcare.
